What Is Family Therapy? Does It Really Help Someone Quit Drugs, and How Should Families Prepare?

Addiction does not only affect the person using; it affects the whole household, from trust and communication to everyone's role at home. That is why the U.S. Substance Abuse and Mental Health Services Administration (SAMHSA) recommends family therapy as part of treatment. This article explains what family therapy is, how it helps, what happens in sessions and how families can prepare.
What Is Family Therapy?
Family therapy is counselling that looks at the family as a whole system, not only the person who uses drugs. A SAMHSA advisory based on its TIP 39 guidance explains that it focuses on roles, relationships and communication patterns in the family. As these patterns change, family members can better support recovery and stop behaviours that might trigger a return to use.
Family therapy is not about finding someone to blame, and it does not mean the family caused the addiction. The goal is to make home a place where recovery can continue.
Why Family Matters in Recovery
SAMHSA notes that positive family support is linked to long-term abstinence, while conflict at home and pressure to use are linked to a higher risk of relapse. Involving the family also helps people enter treatment and stay in it.
Families often respond out of love in ways that unintentionally keep the problem going, such as paying debts, covering up, or arguing every time they meet. Family therapy helps them see these patterns and find new ways to respond.
Forms of Family Therapy
SAMHSA describes several levels of family involvement, from attending educational sessions about addiction to structured family counselling and multi-family groups.
For married or cohabiting couples there is Behavioral Couples Therapy, which works on the couple's interaction patterns to support abstinence.
As for who counts as family, SAMHSA recommends letting the person in treatment decide, so it can include close friends or a "family of choice", not only blood relatives.
What Happens in a Session
Typically the therapist helps each person describe what has happened from their point of view, practise communicating without blame, agree on house rules and boundaries, and plan for high-risk situations after the person comes home.
SAMHSA advises that therapists should not keep one member's secrets from the others. Information shared privately is usually brought into the next session, except disclosures of violence or abuse, which are handled separately for safety.
As the person recovers, roles at home often shift; for example, someone who carried everything may need to let go. Family therapy helps everyone adjust to these new roles.
When Family Therapy Should Wait
SAMHSA states that family therapy is not appropriate while the person is going through withdrawal or while there is ongoing violence or abuse. In these cases safety and physical symptoms come first, and family therapy starts when everyone is ready.
Family Therapy at Petch Nava Hope Care
In Petch Nava Hope Care's 28-day program, family therapy is part of Phase 4 (days 22 to 28), the stage that prepares for going home, alongside family communication skills and planning for risks and triggers. After the program there are three more months of follow-up.
Families who want to prepare can start by thinking about how they would like home to change, writing down things they have wanted to say but never dared, and looking after their own wellbeing. If you have questions, our team is happy to talk.
Frequently Asked Questions
How is family therapy different from family education?+
Education helps families understand addiction and treatment; family therapy works on roles, relationships and communication at home. The two can be used together.
What if some family members refuse to take part?+
It can still work. SAMHSA notes that therapists can discuss the absent person's perspective or, with permission, contact them during a session. Starting with those who are ready is still worthwhile.
Does family have to mean blood relatives?+
No. The person in treatment decides who their family is, which can include a partner, close friends or a family of choice.
When does family therapy start?+
Not during withdrawal or while there is violence at home. In Petch Nava Hope Care's 28-day program it is part of Phase 4, before going home.
